Health authorities in Castilla-La Mancha have confirmed the region's first case of Crimean-Congo hemorrhagic fever (CCHF) in Ciudad Real, marking a significant development in Spain's ongoing battle with emerging infectious diseases. The infection was detected in the Puertollano health district, where a 46-year-old man tested positive and was subsequently transferred to the High-Risk Infectious Diseases Unit at Hospital Gómez Ulla in Madrid. This is the first time a case has been officially recorded in Ciudad Real, according to the National Epidemiological Surveillance Network.
CCHF is a viral disease primarily transmitted by ticks, with a mortality rate ranging from 5% to 40%. While severe illness is more common in patients over 60 or those with additional risk factors, the infection can affect adults of any age. The growing presence of virus-carrying ticks in Spain has heightened concerns among public health experts, who note that the number of ticks testing positive for the virus continues to rise each year. This trend increases the potential for human infection, although the overall risk remains low for the general population.
Antonio Mas, a researcher at the Institute of Biomedicine at UCLM and professor at the Faculty of Pharmacy, points out that ticks are not only vectors for CCHF but also transmit a range of other diseases. He emphasizes that, despite the growing prevalence of infected ticks, the likelihood of contracting the virus after a tick bite is still very low. Nonetheless, Mas advocates for greater public awareness and recommends practical precautions: wearing long-sleeved shirts and trousers—preferably white to spot ticks more easily—when in rural areas, and thoroughly checking for bites after returning indoors.
So far in 2026, Spain has registered three confirmed cases of CCHF, including two earlier incidents in Salamanca, Castilla y León. The virus was first detected in Spanish ticks in Cáceres in 2010, with the first human cases diagnosed in 2016. Since then, sporadic cases have been reported, totaling 19 by December 2025, with two in Castilla-La Mancha. In 2024, a 74-year-old man died after being infected by a tick in his garden in Buenas Bodas, Toledo, and another case was confirmed in 2025 involving a 50-year-old man in the same province.
Spain's experience with emerging health threats is not limited to infectious diseases. For example, the country has also faced sudden infrastructure emergencies, such as when Barcelona authorities evacuated four buildings following the discovery of a large sinkhole. These incidents highlight the importance of rapid response and public vigilance in the face of evolving risks.
CCHF is considered a notifiable disease in Spain, requiring immediate reporting and strict infection control measures. The virus is endemic in parts of Africa, Asia, Eastern Europe, and the Middle East, but its presence in Spain underscores the need for ongoing surveillance and public education. The main preventive strategies focus on minimizing exposure to ticks, especially in rural and agricultural settings, and ensuring prompt medical attention if symptoms develop after a tick bite. The Spanish Ministry of Health continues to monitor the situation closely and updates guidelines as new information emerges.
