Catalonia is changing its colorectal cancer early detection guidelines—now the program will include people up to the age of 74. This decision directly impacts the health of thousands of residents, since the risk of developing tumors is especially high in older age groups. As El Pais reports, the authorities expect that expanding the age range will help detect more cases at an early stage, when treatment is most effective.

Until now, the program was only available to citizens aged 50 to 69. Starting in autumn 2026, it will gradually begin to include those aged 70–74. At the same time, those who have turned 69 will not be excluded from the list—they will continue to receive invitations for screenings every two years until reaching the new upper age limit. According to experts, this approach should help reduce mortality from the most common type of cancer in the region.

Statistics and implications

According to El Pais, about 4,200 new cases of colorectal cancer are diagnosed in men each year in Catalonia, and 2,700 in women. This disease ranks second in cancer-related mortality, behind only lung cancer. In 2025, the number of deaths is forecast to reach 2,264, with men affected more often than women. However, with early detection, five-year survival rates exceed 64% for men and nearly 68% for women.

Expanding the screening program will enable the detection of over 1,200 additional cancer cases annually, most at early stages. This means patients will have a higher chance of complete recovery. Regional authorities are focusing on prevention and early diagnosis to change the troubling statistics and reduce pressure on the healthcare system.

How the program works

The program is straightforward: residents of Catalonia aged 50 to 74 receive an invitation every two years to take a fecal occult blood test. Upon receiving the letter, individuals can pick up a free test kit at a pharmacy, collect the sample at home, and return it for laboratory analysis. If the result is positive, the patient is invited for additional examinations, including a colonoscopy, to confirm or rule out the diagnosis.

This approach allows for the detection of tumors before clinical symptoms appear. According to El Pais, mass screening has helped reduce mortality from colorectal cancer in the region in recent years. Importantly, the program is completely free for participants and does not require complex procedures.

Plans and prospects

The introduction of the new age limits will be phased in, starting from the fourth quarter of 2026. Authorities in Catalonia expect that by 2027, all residents of the region up to the age of 74 will be able to undergo regular screenings. This move has already sparked interest in other regions of Spain, where similar changes are being discussed.

According to El Pais, expanding the screening program is not the only measure planned to combat cancer. In the coming years, Catalonia is expected to launch new educational campaigns and improve access to diagnostic procedures for the elderly.

In recent years, Spain and other European countries have seen a trend toward raising the age limit for participation in early cancer detection programs. For example, in some regions of Germany and France, similar initiatives have already helped to reduce mortality and improve patients’ quality of life. In Spain, such changes are also under discussion in other autonomous communities, which could lead to a revision of national cancer prevention standards.
