A tragedy at Hospital Universitario de Burgos (HUBU) has shaken not only the medical community but all of Spain. Two patients receiving cancer treatment died after being administered a dose of medication six times higher than recommended. The cause was a seemingly simple but fatal error during the drug’s preparation. Medical practice, where every detail can mean the difference between life and death, has once again come under close scrutiny.

The hospital administration chose not to conceal the details: the error occurred during the drafting of the medication preparation instructions. The document, which was meant to guarantee safety, contained a critical miscalculation. As a result, although oncologists prescribed the correct dose and the packaging showed accurate information, patients received a life-threatening concentration.

Consequences of the Error

Both patients developed symptoms of poisoning almost simultaneously. They each arrived at the emergency department displaying severe signs of intoxication. One died within a few hours, while the second passed away several days later. For their families, it was a devastating shock, and for the medical staff—a tragic lesson.

But the story didn’t end there. Three more patients who received the same medication were also affected. One of them remains in intensive care, and his condition is critical. The second is still receiving inpatient treatment, while the third, fortunately, has already been discharged thanks to positive progress.

Hospital response

HUBU management immediately launched an internal investigation. A thorough review was conducted of all oncology drug preparation instructions. As a result, the hospital tightened protocols and introduced additional checks at every stage of medication handling. According to the complex director, such an incident has never occurred here, and he is not aware of similar cases in other Spanish clinics.

Hospital officials emphasize that this was solely a case of human error. Neither staff shortages nor the introduction of new computer systems played a role. The mistake was due to inattention when filling out documentation. Nonetheless, every step taken by pharmacists and nurses will now be subject to even stricter control.

Questions of accountability

From the very first minutes after the tragedy was discovered, doctors and the administration have been in contact with the families of those affected. The regional health department has already begun the process of establishing responsibility and potential compensation. It is still unknown whether the families of the deceased will take legal action, but public outcry is significant.

The incident in Burgos once again raises the question: how well are patients protected from human error in the healthcare system? Even the strictest protocols can’t always prevent tragedy if just one mistake occurs in the chain of actions. This time, the price of error was far too high.
