Burnout doesn't just drain energy—it may also raise the risk of serious health problems. A new study of 92,480 Spanish workers found that those with the highest levels of professional burnout had a much greater estimated risk of developing type 2 diabetes, especially if they were physically inactive or worked in lower occupational classes.
The research, published in Medical Sciences on September 3, 2026, drew on health data from routine occupational check-ups between 2021 and 2023. The study focused on adults aged 18 to 69, excluding anyone with pre-existing diabetes, cardiovascular disease, cancer, pregnancy, or those taking antidiabetic medication, to better isolate risk factors.

Burnout and diabetes risk
Researchers used the Burnout Assessment Tool, a 23-question survey, to classify burnout as low, moderate, or high. They estimated diabetes risk using three established scales: FINDRISC, QDScore, and CANRISK. Across all three, the pattern was clear: higher burnout scores meant higher estimated diabetes risk. Burnout itself isn't a variable in these risk models, but its effect showed up anyway.
For workers with high burnout and low physical activity, the odds of falling into a moderate or high diabetes risk category were 5.6 times higher than for more active peers. Even those who stayed somewhat active saw their risk double if they reported high burnout. The link was stronger in men and in workers from lower social classes. Employees in class III jobs faced nearly double the risk compared to those in class I.
Why lifestyle matters
The study found that physical inactivity and poor adherence to the Mediterranean diet explained about 45% of the observed link between burnout and diabetes risk, with physical activity being the bigger factor. However, because the study was cross-sectional, it can't say whether burnout leads to unhealthy habits or the other way around. There's also some overlap, since physical activity is already part of some risk models.
According to recent reports, if a doctor or nurse in Spain is assigned more than five in-person shifts per month, authorities are now required to intervene and add staff to prevent excessive workload. This regulatory move reflects a broader European concern about chronic workplace stress and its health impacts.
The study did not follow participants over time to see who actually developed diabetes. The findings are based on risk estimates, not new diagnoses, and did not include advanced blood tests like hemoglobin A1c or oral glucose tolerance. Burnout was measured using predefined categories, not individual questionnaire answers, and the risk scales were not adjusted for this specific group.
What this means for workplaces
Adding burnout to the risk models improved their ability to tell apart low- and high-risk individuals, with statistically significant gains in accuracy. Still, the authors warn against adding burnout to routine diabetes screening just yet. Only long-term studies can show whether reducing burnout will actually lower diabetes rates.
The main strength of this study is its size and the consistency of results across different risk tools. It does not prove that burnout causes diabetes, or that treating burnout will prevent it. Instead, it points to a strong possibility: stress at work may be an overlooked factor in cardiometabolic health, especially for people who are already inactive or in lower socioeconomic groups.
As job stress and chronic disease rise in Spain, this research highlights the need for more careful study and targeted action. Employers and policymakers can't ignore the connection between mental strain and physical health. Burnout is more than a personal struggle—it's a public health issue with real, measurable consequences.
