
In Castilla-La Mancha, the career bonus system for healthcare workers is being relaunched after a 14-year freeze. The agreement between the regional government and unions is being signed amid long-standing protests and pressure from the medical community. As El Pais notes, this region remained the only one in Spain where such bonuses were not in effect, leading to an outflow of specialists and decreased motivation.
The career professional bonus system enables SESCAM employees—both permanent and temporary—to receive additional payments for experience, training, teaching, and research activities. The regional authorities plan to enshrine this right in law to prevent a repeat of the 2012 situation, when the payments were frozen by the government of María Dolores de Cospedal as part of broad cost-cutting measures.
Details of the agreement
Under the new agreement, from 2026 to 2028, the region will conduct voluntary campaigns to grant new career promotion levels for all categories of SESCAM staff. A separate procedure is provided for those who took part in the last regular process in 2010, which was halted. The payments will be introduced gradually, and according to union estimates, the first amounts could appear in paychecks as early as January 2027.
Each newly recognized level will bring employees about 230 euros per month. All procedures will be carried out simultaneously for different professional groups to ensure equal conditions and transparency. The authorities emphasize that these are ‘million-euro investments’ and the restoration of rights lost during the period of cutbacks.

Unions call the agreement historic and note that the absence of such payments in the region led to specialists moving to other autonomous communities. Representatives of medical organizations stress that reinstating the bonuses resolves a longstanding conflict and restores motivation for employees. The opposition criticizes the timeline for restoring the system, reminding that more than ten years have passed since the freeze.
